Beyond Blame: What Investigations of Intelligence Failures Can Learn from Aviation Safety

Investigations into intelligence failures are frequently conducted by multiple parties in parallel, and several important cases, their conclusions have been controversial. In addition, investigations minor incidents, such as mishandling of sensitive information or equipment, tradecraft errors, similar events where no major consequences detected, tend to be handled internally the concerned agency, through limited interagency cooperation. This can lead missed opportunities prevent them from developing failures. These issues compared investigative procedures employed aviation safety sector, an independent body is responsible for investigating both with a view specifically develop proactive measures repetition. The conclusion presented that Intelligence Community could benefit studying how handled, order more effective addressing incidents at early stage, avoid controversies events.
